Abhijit Balaji 90d7e55075 feat(prompts): enforce unskippable Turn 1 Topic summary 
- Mandates that the first response of every task/phase MUST contain a Topic summary.
- Ensures Topic precedence over all silence or brevity rules for the initial turn.
- Maintains multi-turn silence for subsequent actions.
- Preserves safety exceptions for critical command explanations.
